Choice Home Health Care
Medicare Certified - Provides Skilled Nurses and Therapies for homebound Medicare clients.
Additional services include:

• Activities of Daily Living Support
• Case Management.

The agency is Licensed by the State of California and is fully Medicare Certified.

Community Caregivers
Provides Private Duty Non-Medical Caregivers for in-home services - Caregivers and aides are employed by our agency with all tax, withholding, insurance and liability handled by the agency. All staff are bonded Caregivers who have passed our stringent Background Review. Each Caregiver is assigned to match the specific needs of each client assistance at home after our Free In-Home Consultation determines the best care management plan. Caregivers are available for both short term and long-from a few hours a week to 24-hour daily care and supervision. Private Duty Services include:

• Professional Care Management
• Light Housekeeping Assistance
• Transportation Support
• Companionship

Community Care
For clients and family members who wish to hire their own in-home Caregivers.
Community Care is a State-Regulated Domestic Referral Agency that gives you the assurance that all aides, Caregivers and domestic personnel we refer to you have passed a full background review and have provided us with personal references which are on-file and verified by our staff. Community Care offers a cost effective option for home caregivers and other related domestic staff. Services can include, but are not limited to, assistance for:

• Transportation
• Errands
• Light Meal Preparation
• Light Housekeeping
• Companionship and more depending on client needs.

Clients should be aware that as a Referral agency, all taxes, liability, withholding and other employment issues are between the client and the caregiver, and are not the responsibility of the Agency.

Senior Care Transitions is an exciting new program that offers full service care management for all senior needs. We specialize in assisting seniors and their families by providing information and direct help to solve problems, resolve conflicts, understand options and develop plans to help clients work through difficult senior life transitions. Program Care Managaers have extensive knowledge about the costs, quality, and availability of all services available in Monterey County. We conduct comprehensive Assessments that cover Legal, financial, medical personality, pet and family issues then develop a responsible plan for the future needs, and recommendations for immediate action items to provide for safety, security and health issues.
