Home health care is a valuable extension to any physician’s practice, and is often the critical factor in reducing hospitalization and re-hospitalization rates. The Care Solutions companies partner with physicians and health care facilities to work collaboratively with you and your staff to both enhance quality of care and improve health care outcomes.

We offer our services to help your practice identify those patients whom would benefit from an immediate intervention to prevent a situation from worsening, thus keeping them out of the hospital. In addition, should a hospital stay be needed, we can help you track your patients in the hospital, and report back to you on the post-discharge level of care they are receiving. In fact, many practices make it a point to tell their patients to ask for any one of the Care Solutions companies, including Choice Home Health Care by name when they enter the hospital or other care facility. By working closely with a home care agency, any practice gains an extra set of eyes and helping hands to aid in the care of their patients.

Skilled services provided by Choice Home Health Care include feedback on the patients’ condition during and after an episode of home care. By using Case Management, one clinician is assigned as the point person on a case, providing a centralized avenue through which all important information flows. This is especially critical in the case of medications. Our Case Manager will provide the physician or physicians involved with a written list of current medications, and create an alert should any potential adverse interactions be identified.

To make sure that your Medicare billing is complete, each month your practice or billing company will be provided with a list of Medicare clients certified or recertified for Choice Home Health Care.

Services provided by Community Care and Community Caregivers include private duty care in the home for assistance with the activities of daily living. And, for those patients who require or want extra help when returning home from a stay in a facility, “Handle with Care” is there for them. Through our “Handle with Care” program we can arrange for your patient to have transportation home, provide a home safety evaluation, pick up medications, fix basic meals, even do light housekeeping, or just be a companion when the family can’t be there.
