Our job is to make your recovery to regain independence a process you understand, a process you take part in. To accomplish this, we use a Case Management approach to help you learn how to function best in your own home.
SKILLED NURSING AND PSYCHIATRIC NURSING
We provide Registered Nurses to help you carry out your personal physician’s care plan. After a careful assessment of your medical and home care needs, we provide professional care while teaching and assisting you in achieving maximum independence. If needed, Psychiatric Nurses provide continuous assessment and management for patients with documented psychiatric conditions. The purpose of psychiatric nursing is to optimize the patient’s potential for independent living.
PHYSICAL THERAPY
We will provide you with physical rehabilitation using Licensed Physical Therapists – in accordance with physician’s orders to help you coordinate a home rehabilitation program to include:

Coverage for Skilled Care Services
The intermittent skilled care services of Choice Home Health Care are typically
covered by Medicare, your private insurance such as Blue Cross or Blue Shield,
or worker’s Compensation, provided that they are ordered by your physician. For
services that are not covered by insurance we can accept private payment, as
well. Our friendly and knowledgeable intake staff can assist you in explaining any
co-pay or deductibles your specific insurance plan may have. If you have

EXTENDED CARE SERVICES
There may be times when you will need extra help at home. We provide caregivers who have the experience to help you with hygiene and personal care needs, preparing meals, running errands, and helping you maintain your daily routine so you can maintain a comfortable living environment. Services can be provided for a minimum of two hours up to 24 hours of care dependent on your needs and for your safety, even if you just need to have someone close by during sleeping hours. We can work with you to determine how much care you will need to keep you comfortable in your home.
Coverage for Extended Care Services
Most extended care services are paid for privately as an out-of-pocket expense. Some extended care services may be reimbursed by long-term care insurance. Our administrative staff can assist you with a review of your policy and/or provide information and referral to other resources so that your stay at home is also part of a sound financial plan.
