With just one call to one number, (831) 645-1400, you’re connected to our staff of pre-screened, trustworthy, high-quality, licensed and certified health care professionals whose services are coordinated with your attending physician. Our job is to make your recovery to regain independence a process you understand, a process you take part in. To accomplish this, we use a Case Management approach to help you learn how to function best in your own home.

SKILLED NURSING AND PSYCHIATRIC NURSING
We provide Registered Nurses to help you carry out your personal physician’s care plan. After a careful assessment of your medical and home care needs, we provide professional care while teaching and assisting you in achieving maximum independence. Through professional case management, we help you address all your long term needs. Short-term recuperative care includes recovery services such as:

• Post-operative care
• Wound care
• Ostomy Care
• Dressing Changes
• Cardio-Pulmonary Care and Teaching
• Diabetes Care and Teaching
• Infusion Therapy
• Medication management
• Pain Control
• Catheter care
• Injection training

If needed, Psychiatric Nurses provide continuous assessment and management for patients with documented psychiatric conditions. The purpose of psychiatric nursing is to optimize the patient’s potential for independent living.

PHYSICAL THERAPY
We will provide you with physical rehabilitation using Licensed Physical Therapists – in accordance with physician’s orders to help you coordinate a home rehabilitation program to include:

• Strengthening
• Gait training
• Range-of-motion
• Coordination
• Endurance
• Flexibility
• Exercise Training

They will help you assess your equipment and home safety needs and provide instructions to other caregivers and family members to maximize independent living.

OCCUPATIONAL THERAPY
Licensed Occupational Therapists help you assess and learn to more easily accomplish daily activities such as:

• Dressing
• Home Safety Evaluations
• DME Review and Adjustment
• Exercise Training
• Using adaptive equipment

The goal of Occupational Therapy assistance is to help you return to as independent a life as possible.

SPEECH THERAPY
Fundamental to quality of life are oral functions including speech. We provide patients with Licensed Speech and Language Pathologists to help with:

• Swallowing Difficulties

Our Speech Therapist will also teach family members and caregivers how to help in overcoming any difficulties in communication and nutrition.

HOME HEALTH AIDES
Daily living activities are essential to quality of life. Under the supervision of Registered Nurses or Licensed Therapists, and according to your personal physician’s care plan, we provide you with Certified Home Health Care Aides. They provide help with daily activities such as:

• Dressing
• Grooming
• Meal Preparation
• Light Housekeeping.

MEDICAL SOCIAL SERVICES
The path to independent living can require a psychosocial component. Licensed Medical Social Workers help you by acting as your advocate in care issues. They can help you find and connect to community resources for:

• Financial Assistance
• Respite Help
• In-Home Support Services
• Short-Term counseling
• Long-Term Planning to help you meet your goals for continuing to live at home.

Coverage for Skilled Care Services
The intermittent skilled care services of Choice Home Health Care are typically covered by Medicare, your private insurance such as Blue Cross or Blue Shield, or worker’s Compensation, provided that they are ordered by your physician. For services that are not covered by insurance we can accept private payment, as well. Our friendly and knowledgeable intake staff can assist you in explaining any co-pay or deductibles your specific insurance plan may have. EXTENDED CARE SERVICES
There may be times when you will need extra help at home. We provide caregivers who have the experience to help you with hygiene and personal care needs, preparing meals, running errands, and helping you maintain your daily routine so you can maintain a comfortable living environment. Services can be provided for a minimum of two hours up to 24 hours of care dependent on your needs and for your safety, even if you just need to have someone close by during sleeping hours. We can work with you to determine how much care you will need to keep you comfortable in your home.

Coverage for Extended Care Services
Most extended care services are paid for privately as an out-of-pocket expense. Some extended care services may be reimbursed by long-term care insurance. Our administrative staff can assist you with a review of your policy and/or provide information and referral to other resources so that your stay at home is also part of a sound financial plan.
